Nursing staff do not administer or dispense medications. Therefore, it is important to complete an authorization form in case medication needs to be given to your children. Here are the steps to follow:

Download the medication supply authorization and complete the form.
Send the authorization and the medical prescription issued by the EPS or an authorized health professional to the nursing department responsible for your child's grade.
Once the school approves the authorization and order, they can send the medications listed.
